The Hazard Identification Form is a worksite hazard identification and correction tracking form. It provides a simple way to record observed hazards, assign correction responsibilities, and document completion dates.

The regulation that requires it

T8 CCR §3203 requires every California employer to establish, implement, and maintain an effective Injury and Illness Prevention Program. The standard states in subsection (a)(4) that the program must include procedures for identifying and evaluating workplace hazards, and in (a)(6) that it must include methods and procedures for correcting unsafe or unhealthy conditions in a timely manner. The regulation applies to all employers subject to Cal/OSHA jurisdiction.

Who needs it

California contractors and employers in construction, general industry, and maritime trades need this form. Any employer with employees at a physical worksite in California must maintain an effective hazard identification and correction system under Title 8 regulations. Out-of-state contractors working on California jobsites are also subject to these Cal/OSHA requirements.

What happens without it

Cal/OSHA can issue citations during inspections for failure to identify or correct hazards. Serious violations currently carry penalties up to $18,475 per violation while willful or repeat violations can reach $145,027.
